Claims for Health, Dental or Health Spending Account

The deadline for all claims of health and dental expenses you incurred during any year is by March 31st of the following year. You have 90 days from the end of each benefit year to make a claim. For example, all 2009 health and dental claims, forms and receipts, must be received by Manulife Financial in their office no later than March 31, 2010.

Please complete an Extended Health Care or Dental Claim Form, attaching original receipts, and submit to Manulife Financial at the address printed on the form. If you have put any extra FlexDollar credits into your Health Spending Account, you may use these allocated funds to be reimbursed for eligible expenses like deductibles, or co-insurance or for goods or services paid for according to criteria set out by Canada Revenue Agency. You can print the forms from the LCC Worker Benefits website or call our office at 1-800-588-4226. Remember to complete everything that applies to your claim, attach all original receipts and keep a copy in your file.

As part of your Flex Benefits package, Option 2 under the Medical component, Vision Care includes 80% coverage for prescription contacts to a maximum of $250.00 per 24 consecutive months. Option 3 provides 90% to a maximum of $350.00 for the same period of time. These are some things to keep in mind when making your choices each year during the November re-enrolment period.

If you are in doubt for any reason as to your coverage or if it is a significant cost, do not assume or take someone’s word for it. (For example, orthotics, dental implants, semi-private hospital room, hearing aids, etc.) Please call Manulife at 1-800-268-6195 or get a preauthorization from Manulife who will provide you with details of what is eligible and how much will be reimbursed according to the specifics of the LCC Worker Benefit Plans insurance policies. Revised Health and Dental Forms

The Health Claim form and the Dental Claim form have been revised. These forms are available on this website under the “Forms” tab at the top of each page. Your personal information can be filled out online; however, it cannot be submitted to Manulife electronically. Your signature must be signed by hand after you've completed and printed it. The new claim forms have included an area where you can choose to have your claims conveniently paid directly into your bank account.
